We accept most major insurance plans such as 1199, GHI, Medicare, Blue-Cross, Oxford, HIP, United Healthcare, Fidelis, Magnacare, Aetna, Healthfirst, Affinity, Humana and most unions plans. We do not accept Medicaid as a primary/stand alone insurance. However, we do accept Medicaid when managed by an HMO plan such as Emblem Health Medicaid, Affinity Medicaid, BCBS Medicaid and other managed care Medicaid plans. Please call us if you have any doubts or questionsto confirm if we take your insurance.
We encourage our patients to book their appointments in advance to decrease the number of walks any given day. However, we do try to make accommodations for some emergencies as the scheduling allows but the final decision for walk-ins is left up to the practice manager and staff based on patient flow at the time of request. We now allow for patients to self schedule their appointments conveniently online.
We request 48 hours notice for all referrals and refills as well as 5-7 days for medical records request. Controlled Substance Refills (medications including stimulants for ADHD, benzodiazepines for anxiety, pain medication, prescription sleep aids) require a visit every three months. An up to date Controlled Substance Agreement will also need to be on file. This is for your safety. Other longstanding medications require a minimum of an up to date annual physical, with sooner visits every 3 to 6 months (at an interval determined by your provider) for Diabetes, Cholesterol, Thyroid, High Blood Pressure, Anxiety, and Depression, etc. This is to help ensure adequate dosing and management of your care.
If you are requesting a medication you have not filled in over one year, and you have not been seen for an annual physical, please schedule this physical either at MFM Online Scheduling via DocASAP, by requesting an appointment via our portal or by contacting the office. If the medication needs to be filled prior to this visit, please send a portal message to your provider once you have scheduled the visit. This is for your safety.
We require 1-2 week for lab results for comprehensive labs and diagnostic test. Test may come back sooner than scheduled and patients will be called to go over any lab and test results by any of the providers available.
The majority of laboratory bills are generated due to insufficient/incorrect insurance information. If you do receive a laboratory bill, it is best to contact the laboratory directly and confirm that they have your up to date insurance information. This will most often resolve any issues. For any further assistance, contact our office's billing department during regular business hours.
